the read me file says it best:
StereoQuake is a modification for Quake III Arena in which two similar images are displayed to create a "stereo" effect, similar to Magic Eye books. In other words, the mod allows you to play Quake III Arena with the illusion of three dimensions!

To see the stereo effect in both the game and the screenshots, cross your eyes until you see three images. The illusionary center image will appear to have depth!
Warning: excessive play may cause headaches, sore eyes, and may also make focusing difficult. While the creators highly doubt that any perminant damage can occur. However, any attempts at stereo vision are at your own risk, and the creators of this mod can not be held responsible. Not everyone can see in stereo vision.

As said before, the creators of this mod highly doubt that any permanent damage can result from excessive play. The strain in your eyes is due to the fact that you're exercising muscles you normally don't use. This is just like weight training for your muscles like your bicep. If you rarely use it, 20 pounds will seem like a lot. But if you keep lifting weight, you gain strength and soon you are curling 50 pounds like its a joke. Same with your eye muscles. Most people will probably only be able to play for about 30 minutes before they get a headache at first. But if they keep at, their eye muscles will get stronger and they can play longer and longer, until it's second nature, like breating.

You could even play on homade HMD's using a single LCD panel, just isolate the left and right images for each eye with an internal divider.

They should make a universal stereo driver to display left/right images, it would open the homebrew floodgates.
